Please can we kindly ask our clients to fill in the Request form below and email it with a copy of their Passport and a copy of their Current Medical issued by Aviation Medicals Wessex. This is to ensure that clients data can be correctly verified and processed in concordance with our Data Protection/GDPR policies.

2. Email us a signed copy of your request form below, a scanned copy of your passport and a scanned copy of your latest Medical Certificate issued by Aviation Medicals Wessex
3. You will be sent a payment link to pay an admin charge of £30 to process these documents
Please allow a turnaround time of 28 days to process your application. We will obviously try to send your medical information as soon as we can to your registered email address specified on your MED160 application form and request form.
